42 Yr old female patient elog

This is online E log book to discuss our patient’s de-identified health data shared after taking his/her/guardian’s signed informed consent. Here we discuss our individual patient’s problems through series of inputs from available global online community of experts with an aim to solve those patients clinical problems with collective current best evidence based inputs. This e-log also reflects patients centered  learning portfolio. 

I have done this E log under guidance of Dr Deekshitha.

Case :

A 42 year old female patient presented with chief complaints of nausea, bilateral swellings of lower limbs and decreased urinary output. 

History of present illness :

Patient was apparently asymptomatic 20 days back, then she developed nausea, B/L swelling of lower limbs and decreased urinary output. Then went to hospital and intrude on dialysis. 

History of past illness :

K/c/o of type 2 Diabetes mellitus since 4 years, on medication
No history of HTN, CAD, asthma, tuberculosis 

Treatment history :

History of blood transfusion during dialysis. 

Personal history :

Married
Regular bowels
Normal micturition
No addictions

Family history :

No history of HTN, DM, Heart disease.

General examination :

Pallor +
No icterus, cyanosis, clubbing, lymphadenopathy
Oedma of feet +

Vitals

Temperature - afebrile
Pulse rate - 92/min
Respiratory rate - 18/min
BP- 130/100 mmHg
SPO at room air - 98%

Systemic examination 

CVS system

No thrills
Cardiac sounds are heard
No cardiac murmurs

Respiratory system

No dyspnoea 
No wheeze
Trachea is centrally positioned 
Vesicular breath sounds
No adventitious sounds 

Abdomen 

Scaphoid shape abdomen 
No tenderness, palpable mass
Normal hernial orifices
Non palpable liver, spleen
No free fluid 
Bowel sounds heard

CNS 

Conscious and alert
Normal speech 
No signs of meningeal irritation
Reflexes

53yr male with swelling of face

 A 53-year-old male, a Hotel owner and chief by occupation, The patient presented to the hospital with chief complaints of Swelling of both Legs since 10 days Swelling of face since 7 days HISTORY OF PRESENTING ILLNESS   The patient was apparently asymptomatic 10 days ago.In the first week of June he had a binge of alcohol on occasion of local festival. Later he observed abdominal distension, followed by Bilateral Pedal odema which is pitting type extending up to the knee, insidious in gradually progressive , since one week . A 50 Yr old male patient

This is online E log book to discuss our patient’s de-identified identified health data shared after taking his/her/guardian’s signed informed consent.  Case A 50 Yr old male patient with infected wound on right side of the thigh admitted in surgery referred to medicine i/v/O low hemoglobin  History of presenting illness : Patient was apparently asymptomatic 3 months ago when he met with an accident  Injury to his right thigh Went to local hospital got stitches done over the injury After 1 Month 15 days the wound got infected. Patient was taken to nalgonda.  Drained abscess from the wound Due to spread of infection, incision was extended over the thigh After Around 15 days later he developed anuria 1st dialysis was done in October in view of serum creatinine 2.5mg/dl.  3 dialysis were done.
